Bilateral Abducens nerve palsy in Herpes zoster: A case Report

Abstract

Herpes zoster (shingles) is a common viral infection that results from the reactivation of varicella-zoster virus (VZV), which remains dormant in sensory ganglia after initial infection. We report a case of ocular motor paralysis resulting from herpes zoster. A 17-year female presented to casualty with complaints of fever, headache, rashes and double vision. She had few grouped vesicles involving right side of neck region adjacent to midline above suprasternal notch suggestive of herpes zoster. Extra ocular movement was limited on abduction in both eyes. Both eye anterior segment examination and fundus examination showed normal findings. Cerebrospinal fluid analysis was more likely viral etiology. Magnetic resonance imaging of brain was normal. She was diagnosed of bilateral abducens nerve palsy caused by herpes zoster virus. The skin lesions and ocular movements improved markedly after treatment.
